Document, implement and update the treatment and /or individual service plan (ISP) for clients as assigned.   Write weekly progress notes. Facilitate group counseling sessions including, among others, relapse prevention. Participate in quality assurance review as required by program management. Plan and facilitate aftercare with community-based providers. Identify and develop relationships with community-based providers. Develop and deliver training to program staff about identifying and treating substance abuse in clients
