Care Navigator Case Manager - Adult Services - Brattleboro - 2nd Shift

Fulltime position in the Brattleboro area as a Care Navigator Case Manager for 2nd shift (1-9pm). This role leads service planning and care coordination navigation for adults receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners. Our ideal candidate will have strong collaboration, problem-solving, and interpersonal skills. Bachelor’s preferred plus 2 years of relevant experience. The pay for this position is $49,335-$64,798 annually, which is inclusive of a shift differential.
