As a Case Manager, you'll work directly with older adults and their families to assess needs, develop care plans, connect clients with community resources, and provide ongoing support that helps them remain safe and independent in their own homes. What You'll Do Conduct comprehensive assessments with clients in their homes. Develop individualized care plans based on each client's unique needs. Coordinate services and connect clients with available community resources. Monitor client progress through regular home visits and follow-up contacts. Advocate for clients while maintaining accurate and timely documentation.
